Malignant obstructive jaundice can be caused by cholangiocarcinoma, gallbladder carcinoma, metastatic lymphadenopathy, and pancreatic carcinoma. Although biliary obstruction primarily occurs in advanced disease, it is often the initial presenting sign of malignancy. As such, many of these cancers remain undiagnosed until the later stages of the disease.
